OBTAINING BIRTH RECORDS

After nearly 20 years of having to obtain birth certificates from Trenton, residents can now receive without delay or additional costs.

For those born between 1918 - 2015, Jersey City will issue birth records from a database provided by the State’s registrar’s office. For those born in 2016 and after, Jersey City will use the State’s Vital Record Platform. 

In order to obtain a birth certificate from the City Clerk’s office, residents must complete an application for certified copies. These are available in English and Spanish.

 

By Mail:

If you were born in Jersey City and wish to obtain a certified copy of your Birth Certificate, you may do so via the Postal Service. Simply fill out the REG 37 Form (available here) and mail it in to the address at the top of the form along with a photocopy of your Driver’s License (or some other Government-issued photo ID), a self-addressed stamped envelope and check or money order made payable to the “City of Jersey City”. The cost is $15 for the 1st copy; $5 for each additional copy.


In Person:

If you were born in Jersey City and wish to obtain a certified copy of your Birth Certificate, you may do so in person by requesting an appointment. Simply dial 201-547-5150 to request an appointment. You should bring the completed REG 37 Form (available here) with you to your appointment along with a your Driver’s License or some other Government-issued photo ID. The cost is $15 for the 1st copy; $5 for each additional copy. Cash, check or money order only. Please make your check or money order made payable to the “City of Jersey City”. Appointments are available Monday through Friday from 8:30 AM until 4:00 PM.
